Dry Eye Treatment with RF

RF Informed Consent Form ( FORMA-I)

Radiofrequency (RF) treatment is a safe, modern, in-office procedure for treating dry eyes. At Markham Eye and Vision Care, we offer the Forma-I device for RF treatments. RF treatments use radiofrequency energy to gently increase the skin temperature around your eyes, which helps open up clogged meibomian glands and improve oil flow. This, in turn, helps enhance tear film quality and alleviate dry eye symptoms. RF treatments with the Forma-I device are noninvasive procedures that can help you get relief from dry eyes. PRETREATMENT QUESTIONS: (Please check YES or NO to the following questions)

1. Have you completed/updated your medical history form?
2. Are you taking any new medication, blood thinners, fish oil or turmeric?
3. Are you pregnant or breast feeding?
4. Do you suffer from cold sores or any active infection in the area being treated? ( if Yes, prophylactics may be recommended.)
5. Do you have any current or history of cancer or impaired immune system?
6. Have you undergone recent surgery?
7. Do you have any tattoos in the treatment area?
8. Do you have any pacemaker, internal defibrillator or implants?
9. Do you have any metal pins or plates in the treatment area?
10. Have you commenced using strong actives in the past 2 weeks?
11. Do you have any known allergies?

TREATMENT

Forma/Plus/I Treatment time is approximately 20-60 minutes (please note this will depend on the size of the person and the treatment area). Your skin will be cleaned to remove any excess product, cool Ultrasound gel applied to the treatment site ready for treatment. During the Forma/Plus treatment you will feel a relaxing warm sensation over the skin for approx 5-10 minutes in each area, described similar to a hot stone massage. Throughout the treatment you may notice audible feedback (beeping), this is so the operator can adapt his/her technique to maintain optimal temperature for a comfortable and effective treatment.

POST TREATMENT

I understand post treatment I may experience some side effects and the following has been explained to me. 1. Erythema (redness) is expected for up to 1 hour. 2. Mild edema (swelling). 3. Complications are uncommon and will be managed by the treating clinic. Potential complications are: PIHP (post inflammatory hyper or hypo pigmentation) from hot spot, grazing the skin on bony areas.
